3. Independent Providers
Mosaic Health and Wellness Group is a wellness facility and community host. The practitioners and organizations operating within Mosaic — including but not limited to Alight Family Integrative Wellness and Restore Counseling Services — are independent practitioners and entities. They are not employees, agents, or representatives of Mosaic Health and Wellness Group.
Mosaic Health and Wellness Group is not responsible for and does not supervise, endorse, or guarantee the clinical care, professional conduct, billing practices, outcomes, or advice provided by any independent practitioner operating within its facility. Each provider operates under their own professional licensure, ethical obligations, and legal responsibilities.
4. No Provider-Patient Relationship
Visiting this Site, submitting the contact form, or communicating with Mosaic Health and Wellness Group through this Site does not establish a therapeutic, clinical, or provider-patient relationship between you and Mosaic or any of its independent providers. A provider-patient relationship is established only through a direct, formal agreement between you and the individual provider of your choosing.
5. Contact Form and Communications
The contact form on this Site is intended for general inquiries only. By submitting the contact form, you acknowledge and agree to the following:

Individual practitioners operating within Mosaic who are subject to HIPAA are independently responsible for their own compliance with applicable federal and state privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA). Patients seeking information about how their protected health information is handled should contact their individual provider directly.
7. Intellectual Property
All content on this Site — including but not limited to text, graphics, logos, images, and design — is the property of Mosaic Health and Wellness Group and is protected by applicable copyright and intellectual property laws. You may not reproduce, distribute, modify, or repurpose any content from this Site without the prior written permission of Mosaic Health and Wellness Group.

By attending or registering for any event associated with Mosaic, you acknowledge and agree to the following:
Event attendance does not establish a clinical, therapeutic, or provider-patient relationship with Mosaic Health and Wellness Group or any independent provider operating within its facility.
Mosaic Health and Wellness Group is not liable for injury, loss, or damage sustained by attendees during events held at the facility or at any off-site location associated with Mosaic programming, except to the extent required by applicable law.
Events promoted on the Site that are organized or hosted by third parties are done so independently. Mosaic Health and Wellness Group is not responsible for the content, conduct, or outcomes of third-party events.
Photography and video recording may occur at Mosaic events for marketing and promotional purposes. By attending an event, you consent to being photographed or recorded and to Mosaic's use of such images and recordings in its marketing materials, website, and social media channels. If you do not wish to be photographed or recorded, please notify a Mosaic representative at the time of the event.
Cancellation and refund policies for ticketed events, if applicable, will be stated at the time of registration. In the absence of a stated policy, all ticket sales are final.
